This support service aids participants in implementing their plan by strengthening their ability to connect with wider support systems and comprehend the purpose of funded supports. Support Connection helps participants understand their NDIS plan, facilitates connections with broader support systems, and provides assistance in engaging with service providers. It aims to help participants effectively utilize their NDIS plan, addressing queries as they arise. Support Connection also enhances a participant's ability to maintain existing support relationships, resolve service delivery issues, and independently participate in NDIA processes.

This support service enhances a participant's ability to design and build their supports, with a focus on connecting them to broader support systems in a complex service delivery environment. Coordination of Supports aims to empower participants to direct their lives, not just their services. It focuses on assisting participants in building and maintaining a resilient network of formal and informal supports. This involves collaborating with participants to understand funding, identify expectations from services, and design the support accordingly. Coordination of Supports also includes coaching participants and working with them to develop capacity and resilience within their network.

Specialist Support Coordination employs an expert approach to address specific high-complexity needs or high-level risks in a participant's situation. This support is delivered by qualified and experienced practitioners such as Psychologists, Occupational Therapists, Social Workers, or Mental Health Nurses. Specialist Support Coordination addresses complex barriers affecting a participant's plan implementation and access to appropriate supports. It involves negotiating suitable support solutions with multiple stakeholders and ensuring well-coordinated plan implementation. Specialist Support Coordinators assist in resolving crisis points, ensuring consistent service delivery, and accessing relevant supports during crisis situations.

Specialist Support Coordination is generally provided intensively and for a limited time, dictated by the participant's immediate and significant barriers to plan implementation. Depending on individual circumstances, a Specialist Support Coordinator may design a complex service plan that focuses on resolving barriers and promoting appropriate plan implementation among all stakeholders in a participant's life. After development, ongoing plan monitoring may be carried out by the participant's support workers or other care supports.
